Insurance Forms

First State Orthopaedics participates with most insurances and will gladly file insurance claims for you. Please remember that your health insurance is a contract between you and your insurance carrier and that you are ultimately responsible for payment of your medical bills. Verifying your insurance with our office ahead of time and obtaining any necessary referrals from your PCP will speed up your insurance claims.

Referrals

If you are a new patient to our office and your insurance plan requires a referral, it is your responsibility to obtain one from your primary care physician. Our managed care representatives will assist you in obtaining follow up referrals thereafter.

Co-Pays

Co-pays are due at the time of your visit. Please contact us if payment in full would be a financial hardship, we would be pleased to set up payments arrangements for you.

Missed Appointment and Cancellation Policy

Here at First State Orthopaedics, we recognize that in today’s busy world, adhering to a schedule is important in order to maximize time and meet the demands of daily life. Our goal is to provide quality care in a timely manner and our commitment to your well being is something that everyone in our practice takes quite seriously. Your appointment time has been set aside specifically for you. With this in mind, we have implemented a missed / cancellation policy which enables us to better utilize available appointment times for our patients.

A missed appointment or “no show” is a patient who misses an appointment without cancelling or rescheduling in advance. A late cancellation is a change made to a scheduled appointment that is cancelled or rescheduled without giving a 24 hour advance notice to our staff. With the exception of serious emergencies, it is expected that you keep all your scheduled appointments. Late cancellation or missed appointment fees may be assessed.

Missed appointments and late cancellations are disruptive to our schedule and to our patients. Please be courteous and call First State Orthopaedics promptly if you are unable to attend a scheduled appointment. This allows our staff to reallocate time to other patients who may be in need of urgent treatment.
